Fat burning workouts do not consist of walking on the treadmill for 45 minutes. Plain and simple, out with the truth right up front. Another big myth that I see repeatedly is from people who sign up for fat burning workouts, but do nothing to change their eating habits. I simply can’t emphasize enough the need to incorporate BOTH exercise and good nutrition into your health regimen. It’s really important to go about your nutritional needs the right way. Please do NOT buy into the hype of all these pills and potions promising “safe” weight loss without diet or exercise. Come on, deep down you know that this mentality cannot be good for you. OK, so yes, you do still need to decrease caloric intake (especially wasted junk calories, mmm, junk food) in order to lose fat. I do see people get initial results from a powerful fat burning workout, but then it tends to level off rather sharply at some point, if the dietary changes aren’t implemented.

And what is a fat burning workout, if not cardio? The focus is on muscle building with a high intensity routine. And to give you the biggest result, target the biggest muscle groups. Yes, sometimes just for vanity’s sake, I do bicep curls and that kind of stuff, but legs and back exercises will yield the most results in overall metabolism boosting and fat burning. GU: interstitial nephritis, nephropathy.

Hematologic: anemia, thrombocytopenia, eosinophilia, leukopenia, hemolytic anemia, agranulocytosis.

Hepatic: transient elevations in liver function test results.
Other: hypersensitivity reactions (pruritus, urticaria, rash, anaphylaxis), over growth of nonsusceptible organisms.
INTERACTIONS

Drug-drug. Oral contraceptives: efficacy of oral contraceptives may be decreased. An additional form of contraception should be recommended during penicillin therapy.

Probenecid: increased blood levels of di­cloxacillin and other penicillins. Probenecid may be used for this purpose.
EFFECTS ON DIAGNOSTIC TESTS

Drug produces false positive or elevated results in turbidimetric urine and serum protein tests using sulfosalicylic acid or I richloroacetic acid; it also reportedly produces false results on the Bradshaw screening test for Bence Jones protein.
CONTRAINDICATIONS

Contraindicated in patients with hypersensitivity to drug or other penicillins. It is not recommended for use in newborns
SPECIAL CONSIDERATIONS

Use cautiously in patients with other drug allergies, especially to cephalosporins (possible cross sensitivity), and in those with mononucleosis (high incidence of maculopapular rash).

Before the drug is given, the patient must he asked about previous allergic reactions 10 penicillin. However, a negative history of penicillin allergy is no guarantee against a future allergic reaction.

A specimen for culture and sensitivity lesls should be obtained before giving first dose. Therapy may begin pending results.

ADVERSE REACTIONS

CNS: headache, nervousness, dizziness, somnolence, photophobia, suicidal tendencies, severe depression.

GI: nausea, vomiting, abdominal pain, diarrhea.

GU: impotence, gynecomastia with tenderness.

Hematologic: thrombocytopenia, hemolytic anemia, leukopenia.

Hepatic: elevated liver enzymes, fatal hepatotoxicity.

Metabolic: hyperlipidemia.

Skin: pruritus.
Other: fever, chills.

INTERACTIONS

Drug-drug. Antacids, anticholinergics, H2O blockers: decreased absorption of ketoconazole. These drugs should not be administered until at least 2 hours after ketoconazole dose.

Anticoagulants: effects may be enhanced. INR, PT, and PTT must be monitored and the dose adjusted as needed.

Cisapride: may cause ventricular arrhythmias. Avoid concomitant use.

Cyclosporine: may increase cyclosporine plasma levels. Serum levels must be monitored.

Isoniazid, rifampin: increased ketoconazole metabolism. A decreased antifungal effect may occur.

Paclitaxel: metabolism inhibited. Use together cautiously.

Theophylline: may decrease theophylline plasma levels. Serum levels must be monitored.

Drug-herb. Yew: inhibits ketoconazole metabolism. Avoid concomitant use.
EFFECTS ON DIAGNOSTIC TESTS

None reported.
CONTRAINDICATIONS

Contraindicated in patients with hypersensitivity to drug.
SPECIAL CONSIDERATIONS

Use cautiously in patients with hepatic disease and in those who are taking other hepatotoxic drugs.

Because of the potential for serious hepatotoxicity, don’t use ketoconazole for less serious conditions, such as fungus infections of the skin or nails.

Elevated liver enzymes and nausea that does not subside as well as unusual fatigue, jaundice, dark urine, or pale stool should be monitored; all are signs or symptoms of possible hepatotoxicity.

Note that much larger doses (up to 800 mg/day) can be used to treat fungal meningitis and intracerebral fungal lesions.

Patient teathing

If you have achlorhydria, dissolve each tablet in 4 ml aqueous solution of 0.2 N hydrochloric acid, sip mixture through a glass or plastic straw (to avoid contact with tecth), and finish by drinking a glass of water because ketoconazoJe requires gastric acidity for dissolution and absorption.

Treatment must continue until all tests indicate that acti ve fungal infection has subsided. If drug is discontinued too soon, infection will recur. Minimum treatment for candidiasis is 7 to 14 days; for other systemic fungal infections, 6 months; for resistant dermatophyte infections, at least 4 weeks.

Nausea, common early in therapy, will subside. To minimize, divide daily dose into two doses or take dose with meals
